I found out what happens if they're all tied, and the other tie scenarios, this from Ian Browne at MLB.com

Should the Red Sox, Yankees and Indians all finish with identical records -- certainly conceivable at this point -- Boston and New York would settle the AL East on Monday at Yankee Stadium. The loser of that game would play the Indians on Tuesday. Should the Red Sox and Yankees finish in a tie and the Indians drop out of the picture, New York would win the division (based on having a better head-to-head record) and Boston would be in the postseason as the Wild Card team. If the Yankees win the division and the Red Sox and Indians tie for the Wild Card, the Sox would host the Indians in a winner-take-all game on Monday.

Of course it's going to suck. Welcome to Red Sox Nation. It won't hurt quite enough this year unless they lose to the Yanks in one game playoff.

What happens if the Sox, Yanks, and Cleveland are all tied? That might raise the level of pain sufficiently. It could end up meaningfully baroque enough so that the Sox lose some pair of one game playoffs. Yeah, that would smart.

As much as I loathe it... they can't win every year. You never know, though. Cleveland might help us get to the playoffs. The most evil thing is that the Yankees are doing well. They don't deserve it, damnit.
